Incident Investigation
The word incident is generally associated with safety incidents. Any thing however that is unexpected and out with the norm is also an incident.
Within most organisations there is a process in place to deal with the smaller perhaps more common place incidents. This may be, for example, the RCA process following on from the loss accounting process. In larger organisations there are also investigation teams already in place to react to significant safety incidents which can also be investigated by regulatory bodies.
What do you do though if you are a smaller organisation without those resources available to you, or indeed if it isn’t a safety incident but a significant equipment failure? For example the entire water injection system may not be operational or the cranes may have collided.
Investigation teams from within an organisation occasionally have conflicting priorities or are seen as being too close to the incident. Bringing in either an entire team or filling individual roles from out with the organisation can demonstrate the open and honest culture of the organisation and their commitment to discovering the truth.

- Organising tests to be carried out as required
